Taste Buds wants to make sure you and your family are ok. We hope that you and your family are safe.

This has been a stressful time and communications are still not fully restored. We are attempting to gather contact information and your status to ensure you are safe. Please complete the below form to help us gather your information so we can account for everyone.

Form used to collect data from employees during Hurricane Ida emergency.

  • Please choose your store from the drop down list.
  • Please check off all that apply to you below.
  • Please list the city / state where you are currently staying.
  • Please type in the length of time you will need / intend to stay in the area where you are now (ex: 3 days, 2 weeks, permanently). If you aren't sure, please type in the maximum time you think you would stay.
  • Please type in your current and future housing plans. We understand you may not have it figured out 100%, so please include what you think may be most likely.
  • Please type in the best way for us to continue to communicate with you directly if we need to reach you. You may select all or more than one option.
  • There have been some issues with various cell service providers. Please type in the best phone number to communicate with you.
  • We know that some of you will be anxious to get back to work as soon as possible. Some locations may open sooner than others. Please use the below to help us get a list of who is willing and able to return to work once restaurants begin to re-open.
  • If your home store is not open within the time frame you chose, please check off all other locations where you are able to work. Please choose all that apply.
